The Application Packet Includes the Following
  • Checklist 
    • Please make sure all items on the checklist are included before submitting the application.
  • Application Form
    •  All pages must be completed and returned. 
    • Make sure to use the Navy date format (dd mmm yy). 
    • If a page is not applicable, complete the applicant's name, write N/A in the first box, and sign the back page. 
  • Report of Medical Exam - pages 5 & 6
    • The applicant's doctor will have to complete and sign pages 5 & 6; we cannot accept the standard forms provided by the office.  Most doctors will complete this form if the patient has had a physical in the past 12 months without another visit. Please call your doctor's office to find out their policy.  
    • The doctor's form is in the application and has also been included as a separate file in case the practice has a portal where you can upload it. Please remind them to use the Navy date format (dd mmm yy). 
  • Uniform Agreement

Completed applications can be mailed to the address on the checklist or brought to drill.  Remember to keep a completed copy for yourself
